Five year old mare (life expectancy in horses: about 30 years), with exercise intolerance, holodiastolic murmur with the point of maximal intensity over tricuspid valve, moderate aortic and mitral regurgitation, tachycardic. An echocardiogram showed a 6x6 cm subaortic ventricular septal defect. There was also a 10 cm right sinus of Valsalva aneurysm bulging into the right ventricle. Contrast study from jugular vein: no positive or negative contrast. There was no intracardiac shunting by Doppler. There is no surgical repair possible in the horse. Two days later, the mare was found dead.
